Physical Therapist for home health care in Methuen, MA, and Nashua,
NH. Responsibilities: • Provide quality physical therapy to home
health patients. • Document using HCHB and OASIS, ensuring same•day
submission. • Manage 20•25 cases weekly, achieving 26•28
productivity points. • Attend weekly in•person office meetings. •
Offer flexible care beyond standard hours (8am • 5pm).
Requirements: • Licensed Physical Therapist in MA and NH. • 2 years
of home health experience with HCHB/OASIS. • BLS certification. •
40•hour workweek with flexibility to float. • Reside within 30
minutes of the territory or secure nearby housing. Details: •
13•week contract, possible extension. • No on•call or overtime. •
