Community health center in the Hartford suburbs is actively seeking a Family Practice physician to join their team.


FAMILY PHYSICIAN

• 
  
• All ages primary care including preventive visits, chronic care management
  
• Weekday telephone call up to 5 weeknights a month, weekend call approximately 1 in every 6 weekends, and holiday call once a year
  
• Strong collaboration with Physician Assistants and Advance Practice Registered Nurses

  • Teaching and research opportunities through affiliated academic partners, including academic title and resources

      • Documentation in eClinical Works electronic health record

        • Billing submission at the time of office visit is expected
          
          
          Requirements:
          
          
          • 
            
          • Board Certification or Board Eligibility (for new graduates)
            
          • New graduates of residency training programs encouraged to apply
            
          • MAT (Medication-Assisted Treatment) for Substance Use Disorders certified or willingness to certify as part of orientation
